Evaluation Privacy Policy

This Privacy Policy applies only to participation in the case evaluation. A separate privacy policy for all website visitors can be found here (as linked from the bottom of each web page).
You acknowledge that all of the information collected during case evaluations are publicly summarized on this website for seven (7) days, and permanently deleted after another 30 days. It can only be found with a unique ID CODE that only you know (as shown when you complete the evaluation). Finding your best legal representative occurs by comparing representation offers made by legal representatives who have reviewed your evaluation results, after obtaining your ID CODE either from this website or as shared directly by you.
Participation in the case evaluation itself is anonymous, as it does not require any personal identifiers such as your name, phone number or email address.
It remains however possible that, by combining non-identifying information in your evaluation, someone could conclude, rightly or wrongly, has found your particular case evaluation. However, doing so would require that the person already knows a significant amount of specific information about you and your case to believe can distinguish your results from other anonymous evaluations (including submission date and the region of the country that you reside).
It is then reasonable to assume that an ability of a “bad actor” to link your identify to your evaluation in this way is very unlikely. It is nevertheless recommended to restrict knowledge of your participation in the case evaluation to trusted family and friends, as well as to the prospective legal representatives to whom you may give access to your evaluation’s results (by sharing your ID CODE).
Out of an abundance of caution, we have also implemented measures that prevent the ability to combine non-identifying information to find your particular case evaluation that includes the following.
Gender information is not obtained in this case evaluation.
Age information is limited to how long before you become 50, 55, 60 or 65 depending on your age group of 18 to 49, 50 to 54, 55 to 59, or 60 to 65, respectively (i.e., in less than 3 months, in 3 to 6 months, in 7 to 12 months or in greater than a year).
You are not asked about the specific type of work you have done, instead only the overall proportion of part- versus full-time work, and the physical demands and skill level of your past/current work.
You are not asked to provide the diagnosis, or affected body system, of any physical health disorder.
You are not asked to provide the diagnosis of mental health disorders.
The evaluation uses only multiple choice questions, that is, without any “free text” answers that could result in mistakenly entering information capable of uniquely identifying your particular results.
Third Party Integration
The forms used to conduct the online case evaluation were possible using a third party platform at JotForm, Inc. (“JotForm”), enabling JotForm to place cookies on your computer or device, which are small files or pieces of text that are used for a variety of reasons. Participation in the evaluation constitutes acceptance of all these cookies as described below.
Some cookies are essential to functionality of the evaluation, helping JotForm make the evaluation forms usable by, for example, enabling basic functions like page navigation. The evaluation cannot function properly without these essential cookies.
Jotform also uses cookies that are not essential to the evaluation’s functionality, but help JotForm understand, for example, how visitors interact with the evaluation’s forms, how well the forms are performing, and to enable analytic functions.
Additional non-identifying information (that are not cookies) may be automatically collected by JotForm from evaluation participants that include browser type, network, device type, web page views, the link followed to arrive at representation-offers.com (if any), and your computer or device IP address. Doing the evaluation constitutes your acceptance of allowing JotForm to obtain this additional information.

Of all the above information collected by JotForm, only the IP address is provided to SSD Evaluations, LLC. It is needed to enforce violations of the terms of use to participate in this evaluation.
All the forms used in the case evaluation are served by Jotform across a protected 256 bit SSL connection that uses a SHA256 Certificate, and uses the standard of security as required by major credit card companies (JotForm is a PCI DSS Service provider and Level 1 compliant).
Age Limitation
Use of the anonymous case evaluation is restricted to people who are at least 18 years old. If we become aware that anyone under 18 years old has submitted a case evaluation, we will promptly delete it from our records.
Changes to this Privacy Policy
We reserve the right to modify or update this Privacy Policy at any time, without prior notice. It is your responsibility to make sure you have read and accepted the most recent Privacy Policy, by reference to the version date as shown with your acceptance as required with each participation in the case evaluation.
